New Digital Conflict of Interest and Commitment Disclosure Forms Now Available
The Office of Research Integrity recently launched new digital Conflict of Interest (COI) and Conflict of Commitment (COC) forms, now available via InfoReady. These digital forms replace the previous fillable paper/PDF versions, streamlining the disclosure process.

The BSU COI, BSU COC, and State of Indiana COI disclosure form can be accessed by those with a conflict to disclose here.
If you have no conflicts to disclose and would like this on file with the university, please file your disclosure here.
Disclosure Requirements
As outlined in Section 38 of the Ball State Employee Handbook, all employees are obliged to report any conflicts and encouraged to report any perceived conflicts annually if:
- The individual or their dependent(s) receive financial support from a domestic entity that alone, or when aggregated, totals $5,000 or more over a 12-month period.
- The individual spends more than 20% of their time (one day per week) on activities outside the University.
- The individual or their dependent(s) receive any support, regardless of the amount, from a foreign entity.
Submitting these disclosures helps safeguard both employees and the institution by ensuring transparency and compliance with University policy.
State of Indiana Disclosure for University Contracts and Self-Authored Works:
If you or your dependent(s) will receive $250 or more over a 12-month period from a University contract or from the use of self-authored materials at Ball State, you must complete a State of Indiana COI disclosure. Access to the necessary state disclosure form is provided within the InfoReady form.
State disclosures oblige approval from the Board of Trustees before the materials or services can be utilized. To meet the deadline for the May 2, 2025, Board meeting, we recommend submitting your completed disclosure by March 26, 2025.
