State of Indiana Disclosure for University Contracts & Self-Authored Works

The deadline is approaching for the State of Indiana Disclosure for University Contracts and Self-Authored Works.

If you or your dependent(s) will receive $250 or more over a 12-month period from a University contract or from the use of self-authored materials at Ball State, you must complete a State of Indiana COI disclosure. Disclosing prior to the upcoming Board of Trustees Meeting on June 13th is particularly important if you plan on using self-authored materials for the Fall 2025 term. State disclosures legally require approval from the Board of Trustees before the materials or services can be utilized. The June 13th Board meeting is the last time we will be able to receive Board approvals before the start of the Fall 2025 term.

If you will be submitting a State of Indiana COI disclosure and have not yet done so for the next 12 months, please do the following by Wednesday, May 21st, 2025

  • Complete the state form
  • Acquire a memo of approval from your Dean or their designee
    • This should state how and why they support the activity and how it benefits the institution
  • Send both the form and memo to the Vice Provost for Research, Dr. Stephanie Simon-Dack at slsimondack@bsu.edu
