Dear Prospective Participants,

My name is Danielle Miller, and I am a doctoral student in Counseling Psychology at Ball State University. I am writing to let you know about an opportunity to participate in a study: Relationship between Perceived Physician Empathy and Health-related Quality of Life among Young Adults with Cancer (IRB #1994526-1). I am conducting a research study examining the overall wellbeing and experiences of young adults coping with cancer, and you are invited to participate in the study.

If you agree, you will participate in a 10–15-minute anonymous online survey about your cancer journey. All responses will be kept confidential and de-identified. At the end of the survey, you will have an option to provide your email address for a chance to be randomly selected to make a $10 donation to a cancer organization or foundation of your choice. The donation will be made by the researcher – you are not expected to pay the donation yourself. A total of 10 participants will be selected for the donation. You will not be penalized if you choose to forgo this option.

You are eligible to participate in this study if you meet all of the following criteria:

•       Between the ages of 20 and 39

•       Have any current type of cancer diagnosis

•       Currently receive any type of treatment for cancer in the United States

•       Have unimpaired cognitive skills and comprehension

•       Can read and understand English

You are ineligible to participate in this study if you can answer “yes” to any of the following criteria:

•       Younger than 20 or older than 39

•       Do not have active/ current cancer diagnosis or is in remission from cancer

•       Receive cancer care outside of the United States

•       Do not read or understand English
