Forms will be available online in the Workday system around the end of February 2026. To access the form, follow the steps below:
- Log in to Workday through myballstate.bsu.edu.
- Search for Workday or go to Human Resources Tools.
- In Workday:
- Select Menu – Benefits and Pay.
- Select My Tax Documents.
- Select View next to the 1095-C document.
- Printed 1095-C forms are ONLY available upon request.
