The IRS requires Ball State University, as an employer, to report whether an employee and dependents (if applicable) had health coverage in 2025. Therefore, employees receive Form 1095-C, which details whether they were offered health coverage and for which months.
Forms are available now online in the Workday system. To access the form, follow the steps below:
- Log in to Workday through myballstate.bsu.edu/dashboard.
- Search for Workday or go to Human Resources Tools.
- In Workday:
- Select Menu – Under Personal, select Benefits and Pay
- Select Benefits.
- Select ACA Forms.
- Click on the View/Print button to open your form. Print if desired.
Printed 1095-C forms are ONLY available upon request.
